How to upload your documents
Gather the operative notes — a folder of files is perfect. We accept .txt, .docx, and .pdf (text-based; scanned images should be exported as text first). You do not need to remove patient information — this page does it on your computer.
Include the billed CPT codes (recommended) — one CSV file with two columns: filename, codes. Example row: "smith-tka.docx","27447;22853-59". Separate multiple codes with semicolons; add a modifier with a dash. Codes are attached to each note before anonymization, so your audit compares billed vs. supported automatically.
Press De-identify & Upload — every note is scrubbed in your browser (names, record numbers, dates, contact details become tokens like [NAME]), assigned an anonymous case number (PT-0001…), and uploaded. Anything our safety gate isn't sure about is held back, never stored — you'll see exactly which line to fix.
How do we match audit findings back to real cases if everything is anonymous?
When the upload finishes, two files download automatically:
Match-back key (medcodepro-match-back-key-KEEP-LOCAL.csv) — maps each case number to your original filename. Keep this at your practice. It never uploads; MedCode Pro cannot see it.
Upload results — case numbers and statuses only (safe to share with us).
Our findings reference the anonymous case numbers ("PT-0042 was under-coded"); you use your key to map them to the real claim. We audit blind — you reconcile locally.
Where is my data stored?
Only the de-identified note text and billed codes are stored, in MedCode Pro's access-controlled database, in a partition private to your practice. Original files and filenames never leave your computer. Re-uploading the same folder is safe — duplicates are detected and skipped automatically. Your team can download the practice's de-identified archive anytime with the button above.
